Position Summary

The CARE Manager serves as the primary clinical and service liaison for clients and their families throughout their Home Instead journey. This role is responsible for conducting in-home Care Consultations, developing personalized care plans, performing ongoing Quality Assurance visits, supporting CARE Professionals, and ensuring outstanding client satisfaction.

The CARE Manager also represents Home Instead throughout the community by cultivating referral relationships with healthcare providers, senior living communities, hospitals, rehabilitation centers, and other community partners.

What You'll Do

Deliver Exceptional Client Care

  • Conduct professional in-home Care Consultations with prospective clients and family members.
  • Assess physical, emotional, cognitive, and social needs to develop individualized care plans.
  • Recommend appropriate Home Instead services that support independence, safety, and quality of life.
  • Complete client and CARE Professional introductions to establish successful care relationships.
  • Perform Quality Assurance visits at least every 90 days, or more frequently as client needs change.
  • Monitor client satisfaction and proactively resolve concerns.
  • Complete home safety assessments and recommend strategies to reduce fall risks and improve overall safety.
  • Educate families on available services, care options, and community resources.
  • Advocate for clients while maintaining the highest level of compassion and professionalism.

Support CARE Professionals

  • Partner with Scheduling and Recruiting to ensure the best possible client and CARE Professional match.
  • Communicate client updates, care plan changes, and service expectations.
  • Assist in resolving service concerns quickly and professionally.
  • Encourage, support, and coach CARE Professionals to provide outstanding care.

Build Strong Community Relationships

  • Develop and maintain relationships with physicians, hospitals, rehabilitation facilities, senior living communities, social workers, hospice agencies, and other referral partners.
  • Represent Home Instead at networking events, educational programs, health fairs, senior expos, and community outreach opportunities.
  • Identify opportunities to grow referral partnerships and strengthen Home Instead's presence throughout Northern Colorado.

Ensure Quality & Compliance

  • Maintain complete, accurate, and timely client documentation.
  • Document consultations, assessments, Quality Assurance visits, incidents, and service updates.
  • Maintain confidentiality in accordance with HIPAA and company policies.
  • Ensure compliance with Home Instead standards and applicable state regulations.

Collaborate as a Team

  • Communicate effectively with office staff regarding client needs and service changes.
  • Participate in department meetings, company trainings, and team development opportunities.
  • Assist with incoming service inquiries and office support as needed.
  • Contribute ideas that improve client satisfaction, employee engagement, and operational excellence.

Qualifications

Required

  • High school diploma or GED.
  • Valid driver's license with reliable transportation.
  • Minimum one year of experience in healthcare, senior care, home care, customer service, case management, or a related field.
  • Ability to travel throughout the service area for client visits.
  • Strong computer skills, including Microsoft Office and the ability to learn company software.

Preferred

  • Associate's or Bachelor's degree in Nursing, Social Work, Gerontology, Human Services, Healthcare Administration, or a related field.
  • Experience working with older adults and individuals living with dementia or chronic health conditions.
  • Previous experience in home care or senior services.
  • Knowledge of community resources available to older adults and their families.

Physical Requirements

  • Ability to travel regularly throughout the service area.
  • Ability to safely navigate clients' homes.
  • Ability to occasionally lift up to 25 pounds.
  • Ability to sit, stand, bend, reach, walk, and drive for extended periods.

Schedule

Full-Time | 40 Hours Per Week

Typical schedule:

  • Friday through Monday
  • Occasional evening Care Consultations
  • Participation in the rotating on-call consultation schedule

Schedule flexibility may be required based on client needs and business demands.
